Reverse total shoulder arthroplasty (RTSA) is completely different from traditional unrestricted shoulder arthroplasty. It refers to the spherical joint surface of the shoulder joint prosthesis located on the glenoid side of the scapular joint, while the glenoid cup is located at the proximal end of the humerus. The head of the reverse artificial shoulder joint is above and the glenoid is below, which is exactly the opposite of our normal human shoulder joint. Therefore, it is also called reverse artificial shoulder arthroplasty.

Master Wang, who is in his 60s, experienced inexplicable pain and weakness in his right shoulder three months ago. However, after receiving relevant treatment in the local area, he still did not improve. The symptoms recurred and the nighttime pain was severe, seriously affecting daily life. Preoperative

After admission, after systematic examination, Master Wang was diagnosed with "aseptic necrosis of the right humeral head" (Cruess stage III) and right rotator cuff injury. Subsequently, after detailed discussion of the condition and preoperative preparation, Zhang Chuankai, Chen Qiang, Yang Peilin, and Xu Meng’s surgical team performed a reverse total shoulder replacement surgery for Master Wang. The surgery was successfully completed.

Director Zhang Chuankai of the Joint Surgery Department of Xuzhou Renci Hospital introduced that our human shoulder joint is composed of the humeral head and the joint pelvis. In the case of humeral head necrosis like Master Wang, traditional surgery only replaces the humeral head and preserves the joint pelvis. However, due to the fact that the artificial humeral head and our own joint pelvis are not made of the same material - the joint pelvis is our own cartilage tissue, while the humeral head is made of metal, Therefore, prolonged friction between the two may cause pain, or lead to situations such as joint instability and limited function.

To address this situation, we are now using reverse shoulder joint replacement, where the humeral head is installed in the glenoid position and the glenoid is installed at the proximal end of the humeral head. The advantage of this approach is that it can increase the range of motion of the patient’s shoulder and make the joint more stable, which is also more beneficial for the patient’s return to normal life after surgery.

Trans shoulder joint replacement is the replacement of previously damaged shoulder joints into artificial joints, mainly used for difficult to repair rotator cuff injuries, elderly shoulder joint comminuted fractures, and revision cases that have failed treatment.
